Shielder Ventures LLC, doing business as D&G Quality Roofing, faces two willful violations and proposed penalties of $223,341 after a 21-year-old roofing worker fell about 30 feet through a skylight of a Houston building, the Department of Labor said Wednesday.
Inspectors with the US Occupational Safety and Health Administration found that D&G Quality Roofing allegedly didn’t provide the worker with a personal fall arrest system, according to the department.
OSHA issued the company a citation for the same violation after a November 2021 worksite inspection near Austin, Texas, the Labor Department said.
